iPhone 4 orders start shipping, others get canceled

iPhone 4 order errors persist despite shipments
Numerous readers today said that their iPhone 4 orders have started shipping. The news follows a day after the phones were readied for shipment and will theoretically have the phones arrive by June 23. Apple is more likely to hold any early shipments for a day to coordinate is launch.
However, some readers have also said that Apple and AT&T are cancelling orders. AT&T hasn't provided clues as to the cause of the problem, but at least one message from Apple has explained that the company was "unable to complete the authorization process" and asked customers to visit either an Apple or AT&T retail store to finish the order. Neither company has commented on the problems.
The behavior may reflect earlier system failures where AT&T's system incorrectly attributed more than one order to a single person or otherwise resulted in cancellations. It's unknown if some of these customers still have active orders or if the problems are eliminating orders outright.

Also there are a number of reasons why the orders are being cancelled.
Credit cards declining. ATT credit check failure.
also several folks have noted that they were placing orders on both systems trying to make sure one went through. So they hit Apple and when ATT processes, it thinks they have already upgraded and can't again until June 2012 and cancels it. Or there's not enough money in the back cause of double holds for the pre-auth one is cancelled etc. or vice versa.
